Comprehensive, Trauma-Informed Residential Care for Adults with Serious Mental Illness
Beacon's residential program integrates psychiatric services, individualized care planning, structured daily programming, and 24-hour support — all delivered in a warm, home-like environment in Aloha, Oregon.
Trained Direct Support Professionals provide continuous on-site care — supporting residents through activities of daily living, implementing service plans, and responding to behavioral and psychiatric needs at every hour of every day.
Our PMHNP delivers psychiatric evaluations, medication management, and clinical oversight as the program's designated Psychiatric Treatment Services Provider under OAR 309-035 — coordinating with each resident's outpatient team.
Every resident receives a personalized Residential Service Plan developed collaboratively by the Administrator and PMHNP, in coordination with the resident's CMHP coordinator and external outpatient provider.
Medications are securely stored, accurately documented, and administered per OAR 309-035 standards. Trained DSPs manage medication administration under PMHNP oversight, ensuring adherence, safety, and resident education.
Staff implement evidence-based, trauma-informed behavioral interventions — including de-escalation techniques, crisis prevention strategies, and skill-building approaches in a safe, recovery-oriented environment.
Residents receive structured coaching in self-care, budgeting, meal preparation, time management, and community navigation — building practical skills needed for greater independence.
Consistent with HCBS standards, Beacon supports residents in accessing community resources, maintaining relationships, and engaging in activities of their choosing — community outings, volunteer work, and social connections.
We maintain active coordination with CMHPs, external outpatient providers, hospitals, crisis services, and primary care providers — ensuring seamless transitions and continuity of care throughout each resident's stay.
For residents with co-occurring substance use disorders, Beacon provides relapse prevention support, motivational coaching, and coordination with outpatient recovery services — integrated care for the whole person.
At Beacon, we understand that residents come with established clinical relationships that matter. We don't replace existing outpatient care — we complement it.
We coordinate actively with CMHPs including Washington County Mental Health, outpatient providers like Sequoia Behavioral Health and New Narrative, hospital discharge planners, and primary care teams — ensuring that every care decision is made in context of the resident's full clinical picture.
