Transitions into residential care are often critical and time-sensitive for clients and their care teams. At Beacon Recovery Residence, our admissions process is clear, efficient, and collaborative โ so referrers, families, and residents know exactly what to expect at every step.
We respond to all referral inquiries within one business day.
Adults (18+) with a primary diagnosis of serious and persistent mental illness (SPMI)
Require 24-hour structured residential support and psychiatric oversight
Do not require inpatient hospitalization or a locked/secure residential facility
Eligible for Oregon Health Plan (OHP)/Medicaid residential services, or are private pay
Appropriate for a small, home-like setting with up to five adult residents
Willing to participate in the residential program and individualized service planning
We maintain active working relationships with Washington County and surrounding CMHPs. CMHP coordinators can refer eligible clients and collaborate on Residential Service Plan development and ongoing case management.
We accept referrals for adults transitioning from inpatient psychiatric units, Oregon State Hospital, and crisis stabilization settings โ working with discharge planners to ensure smooth, well-coordinated transitions.
Outpatient clinics and providers who identify clients in need of a higher level of residential support can refer to Beacon directly. We coordinate to ensure service planning aligns with ongoing therapeutic goals.
Case managers working with adults under civil commitment, Aid & Assist orders, or voluntary SPMI services are welcome to contact our admissions team to discuss availability and client fit.
Families seeking residential mental health support for an adult family member may contact us directly. We will guide families through eligibility and the referral process, including working with the appropriate CMHP.
Primary care physicians who identify patients in need of psychiatric residential placement can contact our admissions team to discuss eligibility and initiate the referral process.
Five clear steps from initial contact to successful placement.
Contact our Administrator by phone or email to discuss the client's needs, our current availability, and program fit. We aim to respond to all referral inquiries within one business day.
Submit referral documentation including recent clinical assessments, psychiatric records, medication list, and behavioral history. Our Administrator and PMHNP will conduct a clinical pre-screening to assess appropriateness for our program.
For OHP-funded placements, we coordinate with the referring CMHP to confirm eligibility and secure authorization prior to admission. For private pay referrals, we discuss financial arrangements directly with the family or responsible party.
Upon authorization, we schedule an admission date and complete intake documentation including orientation, resident rights review, house rules, residency agreement, and initial Residential Service Plan development with the resident and care team.
Following admission, our team engages in regular communication with the referring CMHP coordinator and outpatient treatment team โ attending RSP reviews, coordinating treatment updates, and supporting discharge planning from day one.
